Ready to request an appointment? You are so close!
Please take a minute to read our hospital policies and FAQs. When you have finished, you may click on the "Request Appointment" box at the bottom of the page to submit your request. Once your appointment request has been reviewed and scheduled, we will send you a confirmation notice.
If you are bringing a new patient to the clinic, please complete a registration form online after submitting your request. Additionally, if you are bringing a new dog or cat for a wellness visit, we would appreciate your pet's vaccine history prior to your appointment by email (cedarcreekvetclinic@gmail.com) or fax (517-655-5050).
Payment is expected at time of service. Cedar Creek accepts cash, checks, all major credit cards, Care Credit, and ScratchPay. Examination of pet is required before any vaccines are administered or medications are prescribed.
